Someone else suggested uninstall then reinstall the card.
I agree, but not knowing your skill level, would note that you want
to:
1. start with the card in,
2. run the uninstall utility (if there is one) or else use the
"Add/remove programs" to uninstall anything related to the card,
3. go into the Device Manager and delete the card there (if it still
shows up at that point)
4. shutdown and turn off the machine
5. remove the card
6. power back up, let the machine come all the way up at least once
without the card
7. from here follow the manufacturer's installation procedure
