voip solution

hi,
what i'm trying to do is set up a linux machine connected to the
internet and a phone line.
when someone connects to this machine from the internet, it would dial
the number he requests and connect the call (transfer the audio to the
phone from the internet and vice versa).
also, when someone dials in to the machine he would be connected to
the person on the other end which would be connected to the system
through the web.
can anyone point me towards some documentation or software related to
this?
