In an AD environment, clients synch time with the PDC emulator on
a scheduled basis. That schedule is only adjusted if the client
is out-of-synch by 2 seconds or more -- well beyond the "milliseconds"
tolerance level. You may want to schedule time synch-up on an
hourly or less basis to achieve your tolerance levels -- or perhaps
modify the application that requires this to synch up as part of
the transaction process.
If you are worried about latency caused by routing the time synch, IIRC
Windows will try to adjust for this. However, you may want to run
SNTP on the router and have the hosts on your network synch up with
that. Or, you might try using QoS to reserve bandwidth for the SNTP
protocol.
