I normally leave it on all the time, I only reboot it when it gets
really slow, which previously had only been once every other month or
so.

It's a Netgear DG834T, I think. I know for a fact it's a Netgear and I'm
99% sure the 834 is right, I think the rest is correct, but I'm at work
so can't check.

As I said, it's generally pretty solid, it's only the last few days it's
been a bit erratic.

It was up to 704Kbs or something like that, so this does look to be the
case.

Unlike Sky LLU (EasyNet) as their system uses the 10 day training
period. I had a letter from them confirming this when I was moved to the
ADSL2+ product last December. I ended up with the same download speed,
but an increased upload speed.
