How do I create POE adapter for Proxim/Orinoco AP600 ?

It should be PoE compliant (I don't know the exact 802 standard, but it
follows the standard), so you should be able te get one of those. I think
Proxim is also selling them.
Or look at the standard and build one yourself. You might be able to find
some schematics for a device like this somewhere.

Can I inject 15V or it has to be 48V ?
The original power supply is 12V

It won't hurt to TRY 15 volts, but the 802.3af spec is minimum 40 volts, so
it's unlikely you will get reliable operation from 15 volts.
