in log:
Jan 31 10:13:41 my kernel: LIST_DELETE: ip_conntrack_core.c:304
`&ct->tuplehash[IP_CT_DIR_REPLY]'(c65e576c) not in &ip_conntrack_hash
[hash_conntrack(&ct->tuplehash[IP_CT_DIR_REPLY].tuple)].
when I start -j masquerade

Following that, the system completely locks up forcing a hard reboot.

I searched in google and peoples wrote, that they have
LIST_DELETE: ip_conntrack_core on whatever kernels. (2.4.14 for
example)
I found that peoples adviced check memory or processor
but it's wrong track. This is kernel bug - I think.
I noticed it when I change my ip address in
/etc/sysconfig/network-scripts/ifcfg-eth0
and restart serwer.
Probably my users have open connection and
it cause this error.
Jan 31 10:13:41 my kernel: LIST_DELETE: ip_conntrack_core.c:304
`&ct->tuplehash[IP_CT_DIR_REPLY]'(c65e576c) not in &ip_conntrack_hash
[hash_conntrack(&ct->tuplehash[IP_CT_DIR_REPLY].tuple)].
I try restart 3 times, the same error, only c65e576c was another.

When I restore my old ip address in ifcfg-eth0 and restart
everything is OK.

What kernel version are you currently using (uname -r)? If its not
2.4.24, then upgrade to it.

I was seeing a similar problem running Gentoo with a 2.4.23 kernel. I
upgraded to 2.4.25_pre6 and the problem appears to have gone away.
