We have a Windows server 2003 with Terminal Services/App Mode so that several
remote users can login and use it to work on a database. However, we would
like to restrict the desktop so that it is basically a blank screen with only
an icon of the application they will use. How can I do that in Win2k3?

You have terminal services application mode OR you have the option to choose
the aplication that will begin when connect in terminal services. See the
terminal services options when you create the connection.
And finally you have the Group Policy to change that too
