Are Green codes proper pre-qualify tests?

Does a 'Green' code for 500kbps indicate that a proper high-frequency
pre-test for ADSL has been carried out, or just that the line length
estimate from loss is less than 5km?

Are these tests done from the green cabinet or at the exchange?

My exchange has just triggered but many lines in the area are Red even
though the distance by line to the exchange should be in the 4 to
5.5km range. Although those near the cabinet tend to be Green, whilst
those away tend to be Red, there are also Red ones amongst the Green
ones with no intervening Amber.

Genrally no, it's the second, estimated loss is less than 55db (going up to
60db sometime soon-ish allegedly). YOu'll know for sure when you order and
an insertion loss tests is done.

Test is exchange to house. sometimes outside the house if you're not home.
Can be done from either end.

It's the overall loss that counts.
