well there are filters and filters. You could try

If your modem or router gave line statistics it would help - Noise
margin, that kind of thing. Do you connect at 576/288 according to the
modem or slower upstream ?
Have a read at

can you plug the filter into the test socket at the back of the NTE5
and try the modem and phone together there without the extensions
connected. If that works then a filtered faceplate will resolve it.
You don't have any other phone-type devices like faxes, sky boxes,
alarms that dial out - they all need to be filtered.
You could try two filters in series on the phone, the ADSL doesn't
need a filter but you may need an RJ11 to BT adaptor.
Could you not get broadband before Sept 6th ie is your line long or
high loss ? What does the checker at
www.bt.com/broadband say about
available speeds.
