I'm not aware of any inclusive starter packages offering such hardware but
if you want a truely all in one device you may consider buying the SMC
7404WBRA Barricade. Its reviewed here:

well as the features you're wanting, a good thing about this, which the
others don't have, is a built in parallel port print server so you can share
a parallel port printer between several PCs without one "server PC" needing
to be running. I have one and it works perfectly.
You also then have the freedom of easily being able to change ISPs without
having to give the hardware back - it's yours from the outset.
