Direct debits can go wrong, as there are three parties involved, you,
your bank and your ISP.
I had a similar problem with Plusnet when setting up my village hall's
internet service. After many weeks of failed transfers and cheques being
sent in lieu, it was finally sorted. I suspect the bank but you never
get the whole truth in these matters.
