AD users "disappearing"

We are running a mixed environment of Windows Server 2000 and Windows Server
2003 machines in our Active Directory environment. On two separate occasions
now, we have had users just "disappear" out of the Active Directory. One day
they can log in, the next day they can't; the user is completely gone in
Active Directory Users and Computers.

Is this a known issue? Is there a fix?

You may want to turn up account management auditing and directory access
auditing to find out who is responsible.
Users don't just disappear out of AD.
