Access denied suddenly on some folders and files

Problem: Access denied suddenly on some folders and files

Where: Windows 2003 server sp1 member servers- one Application server
and one user File server
Domain servers: Windows NT 4.0 sp6
Workstations: Windows XP sp2
Anti virus program: McAfee VirusScan Enterprise 8.0
Windows updates: Five updates were in the queue. Some might be
installed; some were waiting for the server to shut down to install.
One update was KB908531 which has some issue with HP printer software

What happened: About 9:00 am, 4-21-2006, some users reported that they
could not save data to files in one application. A few minutes ago
everything was working. I tried to open the application from my
workstation to verify it and received an error message "Access
denied", even I was using domain admin rights. I logged on to the
server but still could not access the folder holding the application.
Rebooting the server did not help much. In the folder's property, the
tab "Customize" was missing. The access control list was blank. I
took ownership of the folder and found "domain users" group and
"Administrators" group were no longer in the access list. But a few
users that had "modify" rights came back. When I added the two
groups back and tried to propagate to its child folders, I got several
error messages from some of the subfolders and files that I have to
take ownership first. I picked up those subfolder and files and took
ownership one by one before re-do the propagation. The application
started to work normally.

The strange thing is that some subfolders and files were not affected.
On the same server, another application in the different folder was not
affected either. No one was working on the server locally or remotely
on that day until it happened. The day before, 4-20, about noon time,
we copied a subfolder from another server to this folder but no error
messages were reported by users.

A similar situation on our User File server a month ago: One user
reported she could not delete one subfolder in her mapped H driver
which was a folder in the file server. She was the only user that has
rights to her H drive. I inspected the subfolder property and could
not see the access control list. The only way to delete the subfolder
was to take the ownership

Any one has ideas what is going on?

You definately have a wierd problem. Did you check the even logs to ensure
it did not report anything unusual?
Do you have any type of monitoring to see if someone is being a smart ass on
your network? "Maybe" someone is logging into someone's account and changing
permissions, etc to cause havok on purpose. If you monitor such things, I'd
check it. Also make sure no one has more rights then they should.

Run a full virus scan, check your HDs for faults. This is truly a wierd
issue, I've never experienced permissions just "disappearing".

Louis,

Event logs showed no clue at all. The brand new application server was
in production only for two or three months. But I will set up some
security monitors on the servers, just in case it happens again.
